Instructions for Vasectomy Testing Kits

1) Fill out the patient information card.

2) Collect sample in a condom after a minimum of 2 days and maximum of 7 days of sexual abstinence.

3) Secure lid on collection container and mix well.

4) Validate that the sample is mixed well in the preservative and is not clotted.

5) Place the sample into a plastic collection bag and put it back into the box with the prepaid postage.

6) Seal box and place in the nearest USPS mailbox.
